LASIK Score is a U.S.-based LASIK research and directory platform operated by LASIK Score LLC. We provide educational content, interactive tools, and data services to help visitors evaluate LASIK providers and pricing. This policy covers your use of lasikscore.com, our embedded calculators, public APIs, and any forms hosted via SubmitForm or other managed providers.

2. Information We Collect

We collect the minimum data needed to operate our directory, improve quality, and respond to inquiries:

• Usage data. Standard web logs (IP address, browser/OS, referring URL, pages viewed, timestamps) captured by our CDN and analytics tools for security, abuse prevention, and aggregated reporting.
• Device & event data. Google Tag Manager (GTM) routes events to Google Analytics 4 (GA4), and Microsoft Clarity records interaction heatmaps. These services may receive cookie or local storage identifiers, approximate location, and page metadata.
• Form submissions. Partnership and support forms (submit-form.com) request your name, practice, contact details, and optional notes. Cloudflare Turnstile protects these forms from abuse.
• User-generated content. If you email us feedback or upload assets through future tools, we process the data you voluntarily provide.
• Public datasets. Our LASIK center profiles rely on publicly available business data, licensing registers, and self-reported pricing. We do not collect patient medical records or PHI.

We do not intentionally collect sensitive personal data (e.g., health diagnoses, precise geolocation, government IDs) unless you provide it to us in free-text form. Please avoid sending sensitive medical information via forms or email.

3. How We Use Information

We process information to:

• Operate, secure, and improve our directory, calculators, and APIs;
• Measure engagement and fix usability issues (GA4, Clarity);
• Respond to partnership inquiries and customer support requests;

LASIK Score is a static-first site compiled with Astro and Svelte, deployed via a Node-based workflow, and distributed through a global CDN. The following vendors may receive limited data to provide core functionality:

• Google Tag Manager / GA4 (Google LLC). Event analytics and measurement.
• Microsoft Clarity (Microsoft Corporation). Session replay and heatmaps to debug UX issues.
• Cloudflare, Inc. D1 database (content refresh pipeline), Turnstile CAPTCHA, CDN, and edge security.
• SubmitForm. Secure intake of partnership and support requests.
• YouTube and other embeds. Media players operating under their own terms when you interact with them.

Each provider governs its own cookies and retention policies. Where applicable, we maintain data processing terms or rely on publicly posted processor terms.

5. Cookies & Similar Technologies

LASIK Score uses a light first-party cookie footprint. GA4 and Clarity may set cookies or local storage identifiers to distinguish visits. You can block or delete cookies in your browser; doing so may affect some features but will not block access to static content. Global Privacy Control (GPC) and similar signals are honored where required.

6. Data Retention

• Server/CDN logs: 30-90 days unless needed for security investigations.
• Form submissions: Kept in SubmitForm and our secure CRM as long as needed to respond and for reasonable audit periods (typically up to 12 months).
• Analytics data: Retained per the default GA4 (up to 14 months) and Clarity (up to 30 days) windows.

7. Legal Bases (EEA/UK Visitors)

Where the GDPR/UK GDPR applies, we act as a data controller and rely on:

• Legitimate interests for core analytics and security;
• Consent for any optional tracking or marketing;
• Contract necessity to respond to partner inquiries and provide requested information; and
• Legal obligation for compliance recordkeeping.
